Health Connected is a free, digital health and wellness platform designed to empower your members with personalized insights and tools to improve their health.
Here is everything you need to engage your members and encourage participation.
Through a dedicated campaign web page (available until June 27, 2025), plan members will be invited to complete two key activities:
- Health Risk Assessment (HRA)
A personalized questionnaire that evaluates 14 health factors and delivers tailored wellness guidance. - Health Scan
A quick, interactive facial scan that provides real-time insights into their health.
Both tools are free and easy to access. Member sign-in is required.

1. Invite Your Members

2. Spread the Word Internally
Boost engagement by promoting the campaign across your organization:
- Post the flyer on your intranet or include it in internal newsletters –
- Share the campaign overview with leadership or wellness champions
- Mention the campaign in team meetings or wellness updates

Driving Insights for the Future
This campaign also serves as a testing ground for creative messaging. We’ll explore different motivators—such as “real age,” “face scan,” and “risk taker” themes—to identify what drives the most engagement. These insights will inform a larger, more robust contest campaign launching this fall.
